By submitting this form I certify that, to the best of my knowledge, the foregoing statements given by me are true. I understand that if I am employed, any misrepresentation or omission by me herein will be sufficient cause for dismissal from the service of Canterbury. I also authorise any investigation of the above information for the purposes of verification. I consent to taking any pre-employment physical examination required by the company and such future physical examinations as may be required by the company. I also understand that the playing of gaming machines within the club premises by employees is a dismissible offence. I understand that application forms are held for 90 days. I understand that after this time I will have to re-apply.

Canterbury will not disclose any personal information you supply on this application form or attached documents to any other organisation or person unless legally required to do so. After 90 days, Canterbury will dispose of your employment application, in accordance with NPP4.
